Our team will take comprehensive records and address any concerns or questions you may have. If you are having problems downloading the forms, please contact our office and we will gladly send them to you.
Your First Visit
Your first visit with us will include:
- Comprehensive oral examination
- Full mouth digital x-rays
- Oral cancer screening
- Oral hygiene instruction
- Teeth cleaning
- One-on-one consultation with our dentist to discuss your needs, concerns, goals, and treatment options
During your visit, we will answer any and all questions about your oral health. We will provide a detailed financial consultation to help you with staging treatments to be timed effectively and affordably.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the initial consultation.
Financing Options
We have many financial options available for your convenience. We will discuss fees and financial arrangements before beginning any treatment. We accept Visa, Mastercard, American Express, Discover, and cash payments. We can also provide you with information regarding a payment plan available through CareCredit.
